Be sure to have a valid i.d. or driver's license and your insurance card(s) with you.
It is also a good idea to have an updated list of all your medications. This list should include prescription and over-the-counter vitamins/medications.

Yes, we accept most insurances but that does not mean we participate. Participating with a particular insurance company means that we have an agreement to accept their allowed amount for a particular service. Accepting most insurances means that we will bill your insurance company for you but we do not have any formal agreement with them and you are responsible for any amount the insurance company does not pay. Please be prepared to pay your co-pay, if you have one, when you check in for your appointment.
We do not accept out of state Medicaid plans.
If you have a question regarding your coverage or eligibility, please contact your insurance company.

-
The fee you pay is for a clinical assessment. The provider will review your situation and use their independent clinical judgement to recommend the most appropriate treatment option for you. This may include prescribing medication, recommending over-the-counter medications or home treatments, or recommending a higher level of care for further evaluation (such as an emergency department or specialist).
We cannot offer refunds solely because the provider determines a prescription medication is not the best option, a medication cannot be safely prescribed based on the information available, does not prescribe your preferred medication, or due to issues related to pharmacy medication pickup.

If you think you are experiencing a life-threatening condition, call 911 or seek immediate medical attention at the nearest emergency department.
The emergency room is used for more serious health conditions. These include seizures, loss of consciousness, newborn fevers, uncontrollable bleeding, weakness/numbness on one side, slurred speech, chest pain, difficulty breathing, severe pain, severe burns, and vaginal bleeding with pregnancy. These conditions require more targeted medical care or extended hospital stays.
Always call 911 and don't drive yourself to the Emergency Department if you are having difficulty breathing, shortness of breath, a life or limb threatening injury, or signs of stroke or heart attack.

Superior Walk-In providers will evaluate and treat patients of all ages, including infants. Depending on the presenting symptoms, infants may need to be directed for further evaluation, but our team of clinicians can help to determine the best plan of treatment. While Superior Walk-In will evaluate anyone – regardless of age – who visits one of our centers, our providers may triage infants (6 months or younger) and refer them back to their pediatrician or ER, where they can receive the best possible care.
-
Superior Walk-In treats a wide variety of injuries and illnesses, including sprains, flu, skin ailments, urinary tract infections, sexually transmitted infections, strep throat, and more. Controlled substances are not the preferred primary treatment option for most chronic conditions. As a primary care practice, we do not provide management of chronic pain (pain lasting longer 3+ months) with controlled substances. If your pain lasts longer than expected and is not adequately controlled with non-opioid medications and treatments, we will work with you, various treatment modalities, and appropriate specialists to ensure your pain is addressed.
Controlled substances like:
Narcotic/opioid medications to treat acute pain
Stimulant medications to treat ADD/ADHD
Benzodiazepines to treat various mental health conditions
may be prescribed after a thorough evaluation and confirmed clinical diagnosis.
If controlled substances are prescribed, a Controlled Substance Agreement (CSA) will be reviewed and signed by the patient and provider.
The CSA will further outline details not limited to urine drug screenings, a prescription drug monitoring program (PDMP), and regular face to face follow-up appointments throughout the course of treatment.

If you need to cancel an appointment, we ask that you please let us know at least 24 hours in advance.
